SUMMARY:
Support for Human Development Foundation (SHUDEF) seeks funding to expand its community-based diabetes awareness and home-based care services from Oshimili North LGA to the entire Delta State. Our mission is to reach vulnerable and physically challenged individuals with essential screening, education, and support to reduce avoidable complications and deaths from diabetes.

BACKGROUND:
Diabetes is a silent epidemic in Nigeria, especially among low-income and physically challenged individuals who lack access to consistent medical care. Many patients suffer in silence, and those with complications such as amputations or visual impairment are left without home-based support or critical medications.

SHUDEF has screened 440 persons in Oshimili North LGA and referred over 26 for emergency medical attention. More than 90 physically challenged patients are now receiving home-based care including diabetes control drugs, non-sugary food, and toiletry supplies.

OBJECTIVES:
Conduct 12 community diabetes screening and awareness campaigns across Delta State

Provide monthly home-based care support for 200 physically challenged diabetic patients

Train 20 community health volunteers to support outreach and follow-up

Procure test kits, diabetes control drugs, and basic support items
