  • As a Passionate Advocate for Youth Health Transitions, championing the cause of adolescents and young adults facing chronic illness and disability during their critical transition to adult healthcare is my aim. Having witnessed the challenges young individuals face during healthcare transitions, I became more passionate about making a difference.
